The Opportunity:

As a Family Care Coordinator (Licensed QMHP), you play a crucial role in planning, coordinating, and facilitating services for our clients and their families. You would work closely with internal teams and external agencies to provide compassionate, effective, and holistic care to those under our care. We’re looking for an experienced and dedicated individual who can uphold our standards and ensure the welfare of our clients.

Job Responsibilities:

  • Assess client needs, develop care plans, and facilitate the implementation of the plan.
  • Coordinate with multidisciplinary teams to ensure appropriate support services are provided.
  • Monitor progress and effectiveness of care plan, making adjustments as necessary.
  • Maintain accurate, comprehensive records of client progress and services provided.
  • Provide instruction and guidance to families in managing mental health conditions and improving overall wellness.

Desired Qualifications:

  • A minimum of a Master's degree in Social Work, Counseling, or a related field.
  • Current and unrestricted Licensed QMHP certification.
  • At least five years' experience in case management, counseling, or a related role.
  • Experience working with diverse populations and an understanding of culturally sensitive care.
  • Excellent communication and interpersonal skills with a patient-centered approach.

Nice-to-Haves:

  • Bilingual proficiency, particularly in Spanish or other languages commonly spoken in the community we serve.
  • Experience in a non-profit or community-based organization.
  • Training in crisis intervention approaches such as CPI or de-escalation strategies.
  • Knowledge of federal and state regulations related to mental health services.
  • Experience with electronic health records systems such as Epic or Cerner.
