Founded in 2009, Granite Pathways began as a grass-roots initiative that introduced the "clubhouse" model in New Hampshire, an evidence-based, self-help community, aimed at eliminating isolation and providing hope, dignity, and recovery options for adults with mental illness. The Granite Pathways of today is a multi-faceted, statewide organization that provides an array of services for those struggling with mental illness or substance use disorders.
**Job Title:** Family Peer Support Specialist **Location:** Portsmouth & Rochester New Hampshire **FLSA Status:** Non-Exempt **Position Type:** Full Time **Position Summary:** Family Peer Support Specialists are staff who understand the New Hampshire child protection system works and have experience, personal or professional, with supporting relative caregivers involved within the Division for Children, Youth and Families (DCYF). Family Peer Support Specialists have demonstrated their own ability to overcome adversity and achieve positive outcomes. The successful applicant will also have solid knowledge of community resources to benefit program participants. Family Peer Support Specialists are trained peer professionals that work with relative caregivers and the children in their homes. **Essential Job Functions:**
* Complete field training including shadowing, DCYF CORE training, trauma informed webinars, peer support webinars, language, and stigma training, DCYF 101 training, Ages & Stages ASQ and Nurturing Parenting.
* Provide in home peer support for relative caregivers through evidence based home visiting models.
* Assess child, and relative caregiver needs through evidence based assessment tools.
* Participate in professional development opportunities including Fedcap online trainings and trainings offered for paraprofessional staff.
* Demonstrate an understanding of and an ability to successfully implement concepts presented in orientation and training.
* Offer peer support to relative caregivers through Motivational Interviewing techniques.
* Provide in home and telephonic support to relative caregivers.
* Travel to DCYF offices and other community partner locations as needed.
* Facilitate Parenting a Second Time Around workshop curriculum for relative care and kinship population.
* Administrative work including case documentation, written correspondence and other data entry duties as needed.
* Attend program Staff Meetings and case specific supervision as scheduled.
* Attend DCYF staff meetings, Better Together meetings and other family voice programing requirements.
* Understand and maintain confidentiality in compliance with all relevant state and federal requirements.

Requirements **Education and Experience:**
* High School Diploma or GED.
* Close familiarity with local and statewide resources.
* Knowledge of family court process.
* Experience working with children and caregivers in a personal or professional setting.
* Experience with Substance Use Disorder, Mental Health Services and Child Welfare system (lived or professional).
* A means of transportation to visit family homes and locations around NH.
* MS office intermediate skills in Word, Excel, and Office 365.
