Requirements to Apply:

• Education: Bachelors Degree in Physical Therapy from an accredited program
• Licenses:
• Unencumbered Physical Therapist License in the state of CA
• Drivers license in the state of CA
• Experience : 1 year of experience working with the frail or elderly population, as a physical therapist
• Certifications: BLS or CPR
• Must be willing to work in both the PACE Center and in the participants home as requested - VERY RARELY

Schedule:

• Full Time - Monday - Friday 8a-5p with flexibility to visit participants outside normal hours if requested (rare).
• May be willing to consider Part Time candidates willing to work at least 2-3 days a week.

Pay Rate: $46.00 - $72.00 Depending on experience

As a PACE Physical Therapist, you will play a pivotal role in our Interdisciplinary Team (IDT), providing high quality physical therapy services to our elderly participants. You will assess, plan and implement rehabilitative programs to address mobility issues, improve functional abilities and enhance overall well being. Your expertise will contribute to optimizing the health and independence of our participant, allowing them to live fulfilling lives in their communities.

Responsibilities:

• Participate as a member of the IDT and complete initial, semi-annual, unscheduled and annual assessments. Attend morning updates and report changes in participants' baseline status to appropriate staff on a daily basis.
• Participate in the development and implementation of quality assessment and performance improvement (QAPI) activities.
• Assess the need for adaptive equipment and durable medical equipment; order and facilitate the authorization of equipment
• Provide treatment and develop special programs according to participant needs and review and revise restorative exercises as indicated.
• Participate in meetings and provide education involving team, family, caregivers, nursing staff at the facility and associated facilities as indicated.
• Provide training, education and supervision for Physical Therapy Assistants, and Restorative Aides and all areas of performance with departmental policies and report as required to supervisors regarding issues.
• Provide Orientation and Training in regard to body mechanics and lift equipment to newly hired staff monthly and as requested.
• Complete documentation as required.
• Provide skilled treatments and assessments to participants in their homes as necessary.
• When appropriate, delegate oversight of PT interventions and activities to a Restorative Aide as determined by the IDT on plan of care.
• Use of a personal vehicle may be required to provide therapy services to participants in their home. This will require reliable transportation, car insurance and drivers license. - VERY RARELY
