Overview

Our Award-Winning Client is seeking a compassionate Home Health Physical Therapy Assistant to join their exceptional care team in Springfield, IL. This role involves collaborating to provide patient care plans, supporting patients in the area, and demonstrating skills in time management and technology use. Key Responsibilities

• Collaborate to provide management and delivery of patient care plans
• Support patients in Springfield, IL and surrounding areas
• Consistently meet critical deadlines for charting
• Self-manage time and schedule

Schedule and Shift Details

• Traditional M-F, 8 am - 5 pm

Travel

• Local travel

Qualifications

Required

• Associate degree or higher
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR
• 1+ years of experience in ANY of the following:
  • Acute Care
  • Rehabilitative Setting
• Valid Physical Therapy Assistant license in the state of Illinois (IL)
• Valid Regular Driver License (DL)

Preferred

• 1+ years of Home Health experience
